This coordination occurs through the direct supervision of employees, support services for individuals and ongoing communication with family, community and funding, and licensure sources.

All coordination must be conducted within contract, state and federal regulations, as well as company policies and procedures.

n n A Typical Day For a House Supervisor Includesn n Person-Centered Planning and Individual Support Planning: n Participating in and monitoring the implementation of Person-Centered Plans (PCPs), Crisis Plans, and other care plans.

Designing and implementing support and teaching strategies targeted at assisting individuals with reaching their planned outcomes.

Providing supervision and training to Direct Support Professionals regarding the implementation of care plans.

Revising PCP outcomes as necessary based on data collection, feedback from staff, and direct observation of individuals.

Ensuring that services and support included in care plans are provided while facilitating opportunities for natural support.

Completing monthly outcome reports and providing to the program manager.

n n Client Record Maintenancen Monitoring client records including profile data, emergency contact information, Medicare/Medicaid information, Release of Information forms, consent forms, guardianship paperwork (if applicable), reviews, progress notes, and any medical information.

Maintaining financial records in our financial system, including documentation of all account spending, personal spending, and household spending.

Completing Monthly Health Summary, Monthly Fire Drills, and Monthly Safety Checklist.

Ensuring that nutrition guidelines on care plans and Crisis Plans are followed.

Ensuring that grocery and household shopping is done.

n n Quality Assurancen Maintaining confidentiality of all individuals’ Protected Health Information as mandated by HIPAA.

Notifying the case manager of any incidents and provide oversight for follow-up plans with staff.

Maintaining communication with family members, guardians, staff, and case managers on a regular basis, particularly regarding policies, issues, and concerns.

n n Supervision/Trainingn Coordinating in-service training for caregivers and field staff.

Providing weekly oversight with documentation of the Medication Administration Record.

Completing a monthly staff schedule, ensuring appropriate levels of staff coverage.

Providing direct support by providing services needed to protect the health and safety of the consumer especially in a staffing crisis.

Communicating with the Interdisciplinary Team about any concerns or findings.

Reporting personnel issues to the program director/executive director.

Performing on-the-job training for all support staff.

Performing on-site evaluations for all field staff.

Completing all annual evaluations.

n n Other Job Functionsn Coordinating client benefits including Medicaid, Medicare, SSI, SSA, and Food Stamps.

Securing community resources for clients.

Coordinating client medical needs.

Assisting the program director/executive director in teaching training classes as requested.

Assisting the program director/executive director with on-site investigations as requested for worker injury and client complaints.

Assisting support staff with other duties as assigned and with approval.

n n In order to be eligible for this role, you should have:n A bachelor’s degree in social work, psychology, human services or a related field OR at least one year of experience in the field of developmental disabilities Supervisory experience preferred Professional oral and written communication skills Professional organizational skills n n House Supervisors must comply with state background screening requirements.
