You will support patients by providing patient-centered health education, advocacy, resource navigation, and referrals to other health and wellness services. How you will make an impact: Complete needs assessments with Howard Brown Health patients who may benefit from assistance in accessing medications, testing and treatment supplies, and other relevant medical and social services related to diabetes Provide information, education, and emotional support to patients in a compassionate, culturally sensitive, and appropriate manner. Evaluate patients’ abilities to understand health plans and provide relevant health plan navigation education Assess financial need of patients and facilitate patient treatment financial support; help with applications, especially as it relates to prescription assistance programs Develop and maintain required records, reports, and statistical data Maintain understanding of the most current scientific knowledge regarding the treatment and prevention of diabetes by participating in ongoing continuing education Maintain the highest level of client confidentiality Other duties as assignedQualifications What you will bring to Howard Brown Health: Minimum Qualifications: High school diploma or GED with 3-4 years experience or Bachelor’s degree with 1-2 years experience Experience working in a community-based medical/social services setting Experience and desire to work with diverse communities including Lesbian, Gay, Bisexual, Transgender, and Queer communities Preferred Qualifications: Case management experience Knowledge and experience working with clients living with diabetes and other chronic diseases Bilingual-English/Spanish language proficiency
