You will support patients by providing patient-centered health education, advocacy, resource navigation, and referrals to other health and wellness services.How you will make an impact: * Complete needs assessments with Howard Brown Health patients who may benefit from assistance in accessing medications, testing and treatment supplies, and other relevant medical and social services related to diabetes * Provide information, education, and emotional support to patients in a compassionate, culturally sensitive, and appropriate manner.* Evaluate patients’ abilities to understand health plans and provide relevant health plan navigation education * Assess financial need of patients and facilitate patient treatment financial support; help with applications,especially as it relates to prescription assistance programs* Develop and maintain required records, reports, and statistical data* Maintain understanding of the most current scientific knowledge regarding the treatment and prevention of diabetes by participating in ongoing continuing education* Maintain the highest level of client confidentiality* Other duties as assignedQualifications What you will bring to Howard Brown Health: Minimum Qualifications:* High school diploma or GED with 3-4 years experience or Bachelor’s degree with 1-2 years experience* Experience working in a community-based medical/social services setting* Experience and desire to work with diverse communities including Lesbian, Gay, Bisexual, Transgender, and Queer communitiesPreferred Qualifications:* Case management experience* Knowledge and experience working with clients living with diabetes and other chronic diseases* Bilingual-English/Spanish language proficiency
